We are in the middle of a Global 40 game where Russia bought a Major IC upgrade intending to upgrade one of its two minor IC's, but the system won't let him deploy/build it. The territories seem to meet the requirements for upgrading. Anyone have this experience before, or know why it would prevent the upgrade?

Japan limited on Minor IC's in Global 40?
I'm in a Global 40 game and, as Japan, I have a minor industrial complex in Kiansu and Kwangtung. I purchased a 3rd one for Malaya, but it won't let me deploy it. It gives me the message "No more constructions allowed in Malaya". In fact, it won't let me build it in any other legal zone on the mainland (with the same message). I've owned all the target territories since the beginning of my turn.
I searched the Global 40 and Pacific 40 rules PDFs for a rule limiting the building of minor IC's, but I can't find one that would seem to limit this. Am I missing something?
How can I prevent resetting of my email server?
My friends and I play by email. Every time I receive a game file and load it, it resets my email server information. It does not reset my email account login and password - just the Host, Port, and Use TLS Encryption fields.
I've tried checking and unchecking the Remember Me checkbox and having my opponents do the same, yet nothing seems to prevent it. It's like the Host/Port info of the last player gets saved into the game file and loaded into the client whenever the next person takes their turn.
Is there anyway to prevent this? It's irritating to have to re-enter my Host/Port info every time I take a turn.
Rule of thumb for defender scrambles?
I recently started playing a Global 40 game, and on turn 1 Germany attacks the British fleet, and the German player asks whether or not he should scramble the British air force in defense of the sea zone. Is there a common rule-of-thumb for making that determination?
I can imagine cases where the British would absolutely scramble and others where they absolutely would not. But for those cases that are less obvious, how do you usually handle them?
- Leave it up to the attacking player's judgement (with them using the "what would I do if I were the defender" rule-of-thumb (but running the risk of making the wrong choice and angering the defending player).
- Stop and email the defending player and ask how they want to handle it (but also slowing the game down).
- Some other method?

Where can I find Map Option Explanations
I am about to begin a game of Global 1940 2nd Edition, and I'm trying to learn if there are any Map Options I would like to tweak. Unfortunately, I can't find anywhere that explains each option, and some of them are not self-explanatory.
When I play the Classic map, the Map Options are explained in the Game Notes under the Help menu, but that is not the case (as far as I can tell) for the Global 40 2nd Ed. map. Am I missing them somewhere?
